Good Faith Estimates

Patients may request an estimate of their charges for non-emergency medical services provided at this facility. Law requires that an estimate will be provided with in five business days of the request. Please call 765-286-7000 ext. 4188 or email estimate@opendoorhs.org to request an estimate.
